ORIGINAL DOCUMENTATION

1. Proof of Employment Eligibility (mandatory)
   One of the following
• Birth Certificate (only if born in the US)
• American Passport
• Resident Alien Card
• Work Authorization Card
• Naturalization papers
2. Social Security Card (mandatory)
3. Proof of Address
• Recent rent receipt or lease
• Utility bill
• Computer printout from other Government Agencies
• Driver’s License or Non-Driver’s ID
4. Proof of Education level or training
• Original HS diploma or GED papers
• Associate, Bachelor or Master's degrees
• Other certifications
• Certificates
5. References
• Two professional letters of reference
• One personal letter of reference

Only when applicable:

6. Name change – if names on documentation are not consistent (one of the following)
• Marriage Certificate
• Legal name change document
• Divorce papers
7. Ticket to Work – for SSI and/or SSDI Beneficiaries
• Original Ticket from Social Security Administration
